What Is Transition Support?
Transition is the move from children's services to adult services, often at the same time as leaving school, starting college or thinking about work and a first home. It can feel like a lot for young people and families to manage at once.
Our transition support helps young adults prepare for and settle into adult life. We work alongside families, schools, colleges and adult social care so support continues smoothly, with the young person's own goals at the centre.
